چکیده

Using an analytical mathematical model, we studied and contrasted the effects of variations in geometry and volume conductor properties of the torso on epicardial and body surface potentials. The model consists of a spherical heart (blood cavity bounded by a spherical muscle shell that includes a double layer source, and pericardium) eccentrically placed in a spherical torso flung region bounded by muscle and fat layers). The effects of the following parameters on body surface and epicardial potentials were studied: (1) separation of the cardiac sources; (2) location of the heart within the torso; (3) combined effects of all torso inhomogeneities, (4) "internal" inhomogeneities (intracavitary blood, pericardium); (5) "external" inhomogeneities flung region, skeletal muscle, subcutaneous fat), and (6) hypertrophy and dilation. It was determined that, although internal inhomogeneities affect both epicardial and surface potentials similarly, the effect of external inhomogeneities on body surface potentials is different from their effect on epicardial potentials. The effects of hypertrophy and dilation are seen to depend on specific details regarding alterations in size and shape of blood cavity, heart, and activation surface. The most important conclusion of the study is that epicardial potential maps accurately reflect the underlying source configuration, are free of the effects of body shape and size, and are affected significantly by only one extracardiac inhomogeneity—namely, the lung region. Such maps, therefore, can enhance our capability to interpret and diagnose electrophysiological events within the heart. Ore Res 46: 283-291, 1980

Forward problem of electrocardiography: is it solved?

BACKGROUND The relationship between epicardial and body surface potentials defines the forward problem of electrocardiography. A robust formulation of the forward problem is instrumental to solving the inverse problem, in which epicardial potentials are computed from known body surface potentials. Here, the accuracy of different forward models has been evaluated experimentally. METHODS AND RE...

Nonsmooth Regularization in Electrocardiographic Imaging

Electrocardiographic imaging is an inverse problem which consists of computing epicardial potential distributions from measurements of body surface potentials. Smoothing and attenuation of the electric field by the intervening body volume conductor render the problem ill posed, requiring regularization to stabilize the inverse solution. This paper reports a study on the efficacy of nonsmooth re...

Noninvasive recovery of epicardial potentials in a realistic heart-torso geometry. Normal sinus rhythm.

The inverse problem in electrocardiography implies the reconstruction of electrical events within the heart from information measured noninvasively on the body surface. Deduction of these electrical events is possible from measured epicardial potentials, and, thus, a noninvasive method of recovering epicardial potentials from body surface data is useful in experimental and clinical studies. In ...

Boundary Element Method in Spatial Characterization of the Electrocardiogram

The electrochemical activity of the heart gives rise to an electric field. In electrocardiography, cardiac electrical activity is assessed by analyzing the potential distribution of this field on the body surface. The potential distribution, or the set of measured surface-voltage signals, is called the electrocardiogram (ECG). Spatial properties of the ECG can be captured with body surface pote...

A model study of volume conductor effects on endocardial and intracavitary potentials.

An idealized mathematical model was developed to study the effects of variations in conductive and geometric parameters on measured endocardial and intracavitary potentials. The model consists of a spherical multielectrode probe located eccentrically within a system of concentric spheres that represent a blood cavity, myocardium, lung region, and surface muscle layer. Solutions were found for e...
